If you take time to examine the deals on offer (they may be time-limited) you can often get connected without an installation fee. When I started, ntl offered free installation if you took the costliest package. I followed ntl's call centre's staff recommendation, took the top package, got the free installation, and at the end of the first month, came down to 150 K/s. No moans. ntl charge me a phone rental of 9.50 a month - which I'd be paying anyway - plus 17.99 a month for always-connected. Being able to use the phone while on-line is a big factor. 17.99 gets you 150 Kbytes/s - 1 Mbyte/s is mind-blowing! In my experience, ntl's call-centre people are very good, right on the button.
